Approval record
An approval record is a document or page that captures the approver, the date, the version, a file fingerprint and the comments that were open at the moment of approval.
Why it matters
It answers the question that starts most design disputes: which version did the client approve? It is a business record, not an e-signature.
Example
The record shows that the client approved v4 on 12 March, with the barcode comment still open and accepted.
